Clinical Observation of Resectable Cases after Preoperative Radiation Therapy in Initially Unresectable Rectal Cancer



Abstract
The over-all survival time from colorectal cancer has been virtually changed in the past two decades, Past, the surgery alone has been standard treatment for rectal cancer.
Despite advances in surgical techique, it dose not afford local control of pelvic recurrence in significant number of rectal cancer and the local recurrence without distant metastasis is still a considerable problem.
To prevent local recurrence and improve the survival, radiation therapy is frequentaly utilized alone or in combination with chemotherapy as a adjuvant treatment.
Reports on combined preoperative radiation treatment and surgical excision in Primary unresectable rectal cancer(eg, frozen pelvis) has been shown the significant number of the tumors removable following radiation.
So, we have analyzed 5 patients with initially unresectable rectal carcinomas treated with preoperative irradiation and later, surgical exploration and resection if feasible, at Dept. of surgery of Koryo General Hospital, between 1970 and 1989.
@ES The results were as follow'
@EN 1) Among the eight patients who had received preoperative irradiation treatment for inoperable rectal cancer, five cases were resectable.
2) The ratio between male and female was 3 : 2, and the mean age was 55.8 years.
3) The pathologic diagnosis were 4 cases of adenocarcinoma and 1 case of transitional cell carcinoma.
4) The preoperative colostomy was performed in 4 cases because of obstruction.
5) The dosage of irradiation were 4500 rad in 3 cases and 6000 rad in 2 cases.
6) In 5 cases who had received preoperative radiation, 3 cases had abdomino-perineal resection and 2 cases had stapled anterior resection.
7) 4 cases had an apparentaly complete resection of their cancer and 1 case did not.
8) 3 cases who have died due to hepatic metastasis(1 case) and carcinomatosis peritoneii(2 cases) had remained cancer cell in pathologic examination, and 2 cases who had not remained cancer cell are alived for 8 years and 16 months.
